The Geriatric Care Management team at VZ-Life provides Verizon employees and their families with a comprehensive adult care support system, including the following:

*in-person assessment of your loved one’s needs in the safety of their home environment.
*Providing you and your family with a care plan customized for your loved one and their needs
*Ongoing care coordination and monitoring.
